SERVICE REQUEST PROCEDURE
 
 

 

Physical Plant

When a request for service, repair or replacement is made with the Physical Plant Department, primarily the Maintenance Department, you should expect a response within three work days from the time your request is received by the department. If your request cannot be met within that time, every effort will be made to notify you of when you can expect to receive service. Availability of materials and parts, labor resources and prioritization may all affect the ultimate response to your request. Maintenance requests are prioritized in the following order: 1) items affecting health and safety; 2) items that affect the immediate operation of classes, laboratories, residence halls, serving of meals, etc.; 3) items that, if not corrected, would lead to serious and costly deterioration; 4) items that are scheduled for a specific date or that have no immediate priority as assigned. Renovations and cosmetic enhancements are not considered maintenance and repair and are performed on a preapproved, preplanned basis.

The following procedure is to be utilized when you require physical plant services:

Students

  1. Complete a service request form. You may do this by using the online maintenance request form or by picking up a paper copy of the form which is available from your RA or Area Coordinator. Please be precise and include all requested information.
  2. Completed service request forms are routed to your Area Coordinator who will serve as your liaison for the service request.
  3. To file electronically from Hollinsnet, click on Forms, Physical Plant Forms, Online Maintenance Request and type in all required information. Click on "Save" and the completed form will be routed directly to the Physical Plant for processing. You will receive confirmation immediately that your request has been sent to the Physical Plant. The date entry clerk will also send you an electronic confirmation once the request has been retrieved, assigned a work order number and sent on to the appropriate department for handling.
  4. In case of emergencies, call the Physical Plant Department directly at extension 6485 during normal business hours. Campus Security is to be called at extension 6419 after normal working hours or in the event you are unable to contact Physical Plant.

Faculty & Staff

  1. Complete a service request form. You may do this by using the online maintenance request form or by picking up a paper copy of the form, which is available from your department secretary. Service request forms are also available from the Physical Plant Department or in the corridor beside the Campus Security office, both of which are located in Botetourt Hall. Please be precise and include all requested information.
  2. Completed service request forms are routed directly to the Physical Plant via campus mail or personal delivery.
  3. To file electronically from Hollinsnet, click on Forms, Physical Plant Forms, Online Maintenance Request and type in all required information. When finished, click on "Save" and the completed form will be routed directly to the Physical Plant for processing. You will receive confirmation immediately that your request has been sent to the Physical Plant.
