Registrar’s Office Forms

Accelerated Three Year Program Request Form  (PDF)    (Word)  Form to Declare Intent to Graduate in Three Years.

Add/Drop Form  (PDF)    (Word)  To be used during the first few weeks of the term to drop or add a course to a previously registered schedule. The form must be signed by the advisor and returned to the Registrar’s Office according to published deadlines.

Arts Management Certificate Form  (PDF)    (Word)  To be complete by students who want to apply for the certificate program.

Change in Student Status Form  (PDF)    (Word)  To be completed by the student and turned into any of the following offices (Registrar, Student Affairs, Business).

Course Block Distribution Form  (PDF)  Used as an aid to schedule classes.

Diploma Information Form  (PDF)    (Word)  To be filled out by potential graduating students for correct information to be included on diplomas and commencement programs.

Enrollment Verification Form  (PDF)    (Word)   Request form that is completed by the student if they need verification of enrollment for insurance companies or other reasons. Bring the completed form to the Registrar’s Office.

ESP Tracking Form   (PDF)    (Word)  Used for tracking a student's progress toward meeting general education requirements (skills and perspectives).

Grade Change Form   (PDF)    (Word)  Faculty can use this form to change an incomplete to a grade.

Graduate Request for Incomplete Grade   (PDF)    (Word)   If a student is unable to complete a course by the end of the term, the student or instructor may request a grade of "Incomplete" which allows the student an extension for one term to complete all coursework. The instructor and Associate Dean must approve all incomplete requests.

Independent Study Application   (PDF)    (Word)  This form must be completed before a student can register for independent study credit. It is turned in with the term registration form at registration.

Interdisciplinary Major Declaration Form  (PDF)    (Word)  To be completed by the student. and signed by appropriate advisors when declaring an interdisciplinary major. Return it to the Registrar’s Office

Internship Agreement Form and Instructions  (PDF)    (Word)  Follow the instructions for setting up and registering for fall, spring or summer internship credit.  

Major Declaration Form  (PDF)    (Word)  To be completed by the student. and signed by appropriate advisors when declaring or changing a major. Return it to the Registrar’s Office.

Minor Declaration Form  (PDF)    (Word)  To be completed by the student, and signed by advisors when declaring or changing a minor. Return form to the Registrar’s Office.

New Abroad Course Approval Form  (PDF)    (Word)  To be completed by the student, and returned to the Registrar’s Office.

New Course Proposal for Graduate Courses  (PDF)    (Word)  Faculty can use this form to submit a proposal for a new graduate course offering.

New Course Proposal for Undergraduate Courses  (PDF)    (Word)  Faculty can use this form to submit a new undergraduate course proposal.

Pass/Fail/Audit Form  (PDF)    (Word)   To be completed by the student and signed by the instructor when a student chooses to take a course on a pass/fail or audit basis. Must be returned to the Registrar’s Office by published deadline.

Perspectives and Skills Course Approval Form  (PDF)    (Word)  Faculty can use this form to apply for one of their courses to meet a general education skill or perspective requirement.

Registration Form - Undergraduate Students   (PDF)    (Word)   Form to be completed by the student, signed by the advisor and turned in during the official registration for classes.

Request for Approval of Courses to be Taken at Hollins-affiliated Study Abroad Programs  (PDF)   (Word)  If a Student wishes to take a  course not previously approved at a Hollins-affiliated college or university then they must complete a separate New Abroad Course Approval form available from the international programs office, the registrar's office.  The New Abroad Course Approval form is also available for download on this page.

Request for Approval of Courses to be Taken at Other Colleges/Universities    (PDF)   (Word)  If a student wishes to take additional coursework at a different college or university (e.g. at home over the summer, semester abroad through another school, etc.) then they must obtain prior approval from their advisor and the Registrar’s Office.

Roanoke College Reciprocal Agreement  (PDF)   (Word)  Form to be used by a student requesting to take courses at Roanoke College through the exchange agreement.

Short Term Internship Information Packet  (PDF)  Follow the instructions for setting up and registering for short-term internship credit.   Please visit the Career Center for additional information.

Summer Reading Application  (PDF) (Word)   To be completed by the student by published deadlines, if she wishes to earn two semester credits for summer reading work.

Thesis Application  (PDF)  (Word)    To be completed by undergraduate students doing thesis work in their senior year.

Transcript Request Form  (PDF)    (Word)   Formal written request for release of the student’s record in the form of an official transcript. Must be signed by the student and returned to the Registrar’s Office with $5.00 fee per transcript.
