CSA Crime Reporting Form

This form is to be used by “Campus Security Authorities” of Hollins University to report confidential crimes reported to them, and to report Judicial Violations for Clery specific crimes.  These forms will be used by the Director of Campus Safety to annotate the Daily Crime Log and to complete crime reporting requirements under the Clery Act.

Reimbursement Form

This form is to be used by students wanting reimbursement for parking decals.  Please see the Reimbursement Policy here.

Parking Ticket Appeal Form

This form is to be used by all persons wishing to appeal a university parking ticket.  Please see the Parking Ticket Appeal Policy here.

Complaint Against Employee Form

Persons may use this form to file a formal complaint against personnel of the Department of Campus Safety.

Vehicle Registration Form

Students may use this for to register their motor vehicles.  Please see the Registration Policy here.

Firearms Registration Form

Employees living on campus must use this form to register all firearms that will be stored either in their university homes or their private vehicles.  Please see the Weapons Prohibition Policy here.

Driver Application Form

Employees or students wishing to drive university owned vehicles must complete and turn this form along with a photo copy of your drivers license to the Director of Campus Safety.   Please see the University Van Policy here.
